Advanced Clinical Practitioner

Description:

Tameside and Glossop Integrated Care NHS Foundation Trust is seeking an Advanced Clinical Practitioner (ACP) to join its rapidly expanding Same Day Emergency Care (SDEC) unit. This part-time role offers 17.5 hours per week, with a flexible shift pattern typically consisting of two 10-hour clinical shifts one week and one 10-hour clinical shift the following week. Remaining hours are allocated for personal development and unit advancement.

Key Responsibilities:

• Provide expert clinical care across the urgent care pathway, including history taking, clinical assessment, diagnosis, treatment, and evaluation

• Act as a role model of advanced clinical practice

• Maintain high standards of documentation and contribute to the development of electronic patient records

• Ensure effective discharge planning, health promotion, and continuity of care

• Collaborate with nursing, medical, and paramedical colleagues for optimum patient care

• Participate in audit and quality assurance activities to improve clinical practice

• Develop and implement care pathways and promote evidence-based practice
