Description:
Tameside and Glossop Integrated Care NHS Foundation Trust is seeking an Advanced Clinical Practitioner (ACP) to join its rapidly expanding Same Day Emergency Care (SDEC) unit. This part-time role offers 17.5 hours per week, with a flexible shift pattern typically consisting of two 10-hour clinical shifts one week and one 10-hour clinical shift the following week. Remaining hours are allocated for personal development and unit advancement.
Key Responsibilities:
Provide expert clinical care across the urgent care pathway, including history taking, clinical assessment, diagnosis, treatment, and evaluation
Act as a role model of advanced clinical practice
Maintain high standards of documentation and contribute to the development of electronic patient records
Ensure effective discharge planning, health promotion, and continuity of care
Collaborate with nursing, medical, and paramedical colleagues for optimum patient care
Participate in audit and quality assurance activities to improve clinical practice
Develop and implement care pathways and promote evidence-based practice
