Community Health Nurse

Bromley Healthcare CIC is seeking a dedicated Band 5 Community Health Nurse to join their health visiting team in Greenwich. The role focuses on supporting children and families within a defined community, delivering care packages following assessment by a Health Visitor, and contributing to the Healthy Child Programme (0-5 years).

This position offers high-quality exposure and training, preparing the candidate to apply for the Specialist Community Public Health Nursing (SCPHN) programme in September 2026. Note that acceptance into the SCPHN programme is not guaranteed and requires a formal application and interview process.

Key Responsibilities:

• Support delivery of the Local Health Visiting offer, including the Healthy Child Programme

• Work as an independent practitioner within a skill-mixed team

• Undertake delegated work in a timely manner

• Identify children and families with additional needs and refer to appropriate agencies

• Provide health advice and signpost to local community resources

• Conduct developmental assessments and 6-8 week reviews when trained

• Deliver health promotion activities and targeted interventions

• Adhere to Safeguarding protocols to protect children from harm or neglect

• Carry out early intervention, safeguarding, and multidisciplinary health referrals

• Perform home visits to children and their families according to health objectives

• Liaise with GPs and Primary Care Teams as required
