Description:
The Role
As a valued member of our Reach Team in London, you'll be taking on a central role with responsibilities including
- Work in close partnership with Accident & Emergency teams to develop and implement care plans for frequent hospital attenders.
- Providing specialist assessment of patients attending/admitted to hospital with alcohol related issues using AUDIT screening tools and to provide brief interventions and onward referral for patients.
- Providing advice, support and training to hospital staff regarding drug and alcohol awareness, screening, assessment processes and referral pathways.
- Developing and facilitating education/training opportunities to enhance the knowledge and skills of clinical staff working with this group.
- Working closely with community substance misuse teams to ensure assertive and effective follow up of repeat presentations and discharges from the hospital.
- Keeping GP's informed of patient’s status and discharge plan/referrals within 5 days of patient’s hospital discharge.
- Developing specific links with identified specialist acute services.
About You

- The ability to manage challenging behavior in a calm and professional manner.
- The ability to deliver bite sized training sessions to professionals.
- Experience of working sensitivity in a hospital setting.
