Mental Health Practitioner

Description:

Greater Manchester Mental Health NHS Foundation Trust is seeking a Band 6 Registered Mental Health Nurse to join urgent care services at the Salford Home Based Treatment Team and Listening Lounge. This 24/7 service provides crisis support in the community, offering an alternative to psychiatric inpatient admissions. The Listening Lounge is a friendly, supportive environment for urgent mental health assessment and brief psychosocial interventions, ensuring timely care without long emergency department waits.

Key Responsibilities:

• Screen and gatekeep referrals, ensuring the least restrictive treatment options

• Conduct comprehensive mental health and social needs assessments

• Develop and manage collaborative care plans for community treatment

• Perform risk assessment, formulation, and management of service users

• Act as keyworker, coordinating care with the multi-disciplinary team

• Cover duty/shift coordination roles when required

• Liaise with other agencies involved in service user care

• Lead discharge or transfer of care to appropriate agencies

• Support and supervise junior staff

The role requires flexibility to work shift patterns, including nights, as part of the 24/7 service.
