Description:
Danone is looking for a Nutricia Homeward Nurse to join its dedicated healthcare team supporting patients in Cambridgeshire and surrounding regions. As part of Danone’s Nutricia division, this role is central to delivering exceptional home-based care for patients receiving enteral (tube) feeding. You’ll work closely with carers, healthcare professionals, and dietitians to ensure patients receive the highest standard of training, care, and support within their own homes.

As a Nutricia Homeward Nurse, you will provide high-quality clinical training and hands-on care for both adult and paediatric patients who are tube-fed at home. You’ll play a vital role in ensuring smooth hospital discharges, reducing readmissions, and supporting patient independence through education and ongoing clinical reviews.
Deliver comprehensive training to patients, carers, and healthcare professionals (HCPs) to support safe hospital discharge and continued home care.
Conduct regular patient reviews, providing ongoing support, troubleshooting, and clinical assessments.
Assist in preventing unnecessary hospital readmissions, including performing tube changes and emergency care in the community.
Work collaboratively with dietitians, GPs, and local nurses to ensure a consistent, high-quality standard of care.
Build and maintain strong relationships with patients and HCPs, serving as a trusted point of contact for all homeward care matters.
Manage your schedule independently while ensuring all visits and follow-ups are effectively planned.
Registered Nurse (RGN, RSCN, or RNLD) with valid NMC registration.
Minimum of 12 months post-registration nursing experience.
In-depth understanding of the UK healthcare system and NHS operations.
Strong communication and interpersonal skills to engage patients and professionals effectively.
Ability to work independently and manage a dynamic field-based workload.
Must be living in or willing to relocate to the Cambridgeshire region.
Full UK driving license required.
