As our Support Coordinator, you’ll play a really important role in supporting the smooth running of our service. You’ll be working with our young people who are aged between 16–25. We help them in the areas of emotional support, building on their independent living skills, provide advice and we advocate for our customers. Many of our customers have a mental health diagnosis and can present with behaviours that are challenging. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations.

Typical day as a Lead Support Worker (known by us as Coordinator)
 

  • Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans
  • You’ll be assisting our fantastic customers with complex mental health
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Working closely with external agencies so that there is a seamless approach to support.
  • As a Support Coordinator, you are supporting our Support workers in service delivery as well as focusing on our service KPIs and working alongside management to achieve these.

You bring
 

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues with this.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • Experienced in safeguarding, risk assessments, confidentiality and data protection.
  • IT skills: It's important that you are technologically savvy (or at the very least not a technophobe!)
