Homeward Nurse

Description:

Danone is seeking a Nutricia Homeward Nurse to provide training and care for patients receiving tube feeding at home. The role involves building strong relationships with carers, healthcare professionals (HCPs), dietitians, GPs, and local nurses, ensuring safe and effective patient care.

Key Responsibilities:

• Deliver comprehensive training to patients, carers, and HCPs to ensure safe hospital discharges and ongoing home care.

• Conduct regular patient reviews and provide support for troubleshooting and queries.

• Assist in preventing unnecessary hospital stays, including performing tube changes within the community.

• Foster strong relationships with patients, carers, and healthcare professionals to enhance quality of care.

Candidate Requirements:

• Registered Nurse (RGN, RSCN, or RNLD) with at least 12 months of professional experience.

• Reside in the designated area (East Suffolk & North Essex).

• Full UK driving license.

• Ability to work independently while effectively managing your schedule.

• Strong understanding of the UK healthcare system, including NHS operations.

• Excellent communication skills for engaging with patients, carers, and healthcare stakeholders.
