Occupational Therapist

Are you an enthusiastic and experienced Occupational Therapist or Physiotherapist looking to develop your clinical skills within a dynamic community setting?

We are seeking a motivated Band 6 therapist to join our friendly and supportive Community Therapy Team. This exciting role offers the opportunity to work autonomously across a diverse caseload, delivering high-quality assessment, rehabilitation and interventions to patients in their own homes.

You will play a key role in supporting people to maximise their independence, prevent hospital admissions and facilitate timely discharge from acute services. Working as part of a multidisciplinary team, you will undertake holistic assessments, develop person-centred treatment plans and collaborate with health and social care colleagues to achieve the best outcomes for patients.

The successful candidate will demonstrate strong clinical reasoning skills, experience managing complex patients, and a passion for delivering excellent community-based care. You will also contribute to service development, support students and junior staff, and participate in our seven-day service model.

Person specification

Qualifications

Essential
 

• Good standard of general education including, GCSE English and Maths grade C or above
• A Levels or equivalent qualification
• Degree / diploma in Physiotherapy / Occupational Therapy
• Registered with Health and Care Professionals Council
• Evidence of CPD including recent post graduate training relevant to clinical speciality
   

Desirable
 

• Evidence of leadership training
• Fieldwork Educator's certificate or attendance at Clinical Educators Course
   

Experience

Essential
 

• Demonstrates clinical experience enabling the post holder to manage a caseload of patients with complex needs. This would usually be approximately 12 months and will include experience in Medical, Older People, Trauma and Orthopaedics
• Specialist knowledge and clinical experience across a range of practices, routine and non-routine. Previous / recent clinical experience working within a Community Therapy Service, completing domiciliary visits and admission prevention pathways.
• Previous and on-going attendance at mandatory training
• CPD portfolio
