Leicestershire Partnership NHS Trust is seeking an enthusiastic and experienced Band 6 Physiotherapist to join its dynamic and long-standing Community Therapy Service. This position is a key role in providing essential rehabilitation support across Leicester, Leicestershire, and Rutland, enabling patients to receive care in their own homes and avoid unnecessary hospital admissions.

The Community Therapy Service delivers both urgent and routine therapy interventions. Urgent services aim to stabilize and rehabilitate patients at home, preventing hospital admission, while routine therapy supports longer-term recovery. The service also includes specialist input into High Dependency Discharge to Assess (D2A) beds, helping patients with cognitive impairments, delirium, or mental health needs transition safely back to their homes or into appropriate long-term care arrangements.

You will be working independently, conducting assessments, planning care, and delivering rehabilitation interventions directly in patients’ homes. The role involves close coordination with a multidisciplinary team consisting of Occupational Therapists, Physiotherapists, and Technical Instructors, as well as participating in daily MDT meetings for patient care planning.

Key Responsibilities:

  • Provide patient assessments and individualized rehabilitation in home settings

  • Support patients transitioning from hospital environments (D2A pathways)

  • Work collaboratively within a multidisciplinary team

  • Engage in CPD opportunities, clinical supervision, and student mentorship

  • Travel across service areas as needed
