Description:
We are looking to recruit two enthusiastic, highly motivated Specialist Pharmacy Technicians in exciting new roles working as domiciliary medicines management specialists in the county wide intermediate care teams (ICT) supporting therapists and integrated care workers with medicines related issues in patient's homes.
These roles support Cambridge and Huntingdon or East Cambs and Peterborough ICTs.
You will be working with the MDTs within ICT with support from experienced domiciliary specialist pharmacy technicians as well as being a member of the trust pharmacy team.
This role involves hybrid/lone working at times and travelling to visit patients in their own homes, so the successful candidate needs to be able to work independently, as well as being self motivated and confident in talking to patients.
The ideal candidate will have a flexible approach to the needs of the service and have a strong team work ethic. You must be well organised, able to prioritise your own work and have good communication skills. You need a strong sense of accountability, accompanied by the ability to work under pressure at times.
If you are looking for a new challenge, enjoy variety in your work and wish to be part of a team striving to make a difference to our patients then we want to hear from you.
The purpose of this post is to provide a patient-centred service to support medicines optimisation and safe use of medicines in the Intermediate Care Team (ICT).
The aim is to improve patient outcomes by optimising the use of medicines by patients in the community and support ICT staff with medicines management related issues. This will improve patient care, reduce medication related problems and avoidable hospital admissions and can also reduce drug costs and waste.
It will require a collaborative approach together with the patient and their family, ICT MDT and pharmacy, as well as colleagues in community pharmacy, primary care, health and social care.
The post-holder will contribute to the strategic development of support for medicines management in ICT and the domiciliary setting.
