Rehabilitation Case Manager

Corporé, one of the UK’s largest independent providers of rehabilitation case management services, is looking for a Rehabilitation Case Manager in Nottingham. This role is an exciting opportunity to join an award-winning rehabilitation provider that supports individuals involved in Catastrophic and Multi-Trauma accidents, helping them regain function, independence, and return to work.

The successful candidate will carry out Immediate Needs Assessments (INA) and proactively manage cases, ensuring a goal-focused rehabilitation plan is in place for clients with life-changing injuries.

Key Responsibilities:

• Conduct Immediate Needs Assessments (INA) for clients with complex injuries.
• Case manage clients, guiding them through goal-focused rehabilitation.
• Use excellent communication and clinical skills to support clients in achieving their full potential.
• Work closely with medical professionals, insurers, and employers to coordinate rehabilitation efforts.
• Ensure evidence-based, high-quality clinical service is delivered to each client.

Clinical Profile & Qualifications:

• Professional registration with HCPC, NMC, or equivalent.
• Minimum 2 years post-graduate experience in Complex Orthopaedics, Multi-Trauma, or Poly-Trauma.
